There are only 2 ways we will process your order:
#1 We leave the "To" and "From"
fields blank and mail it directly to you to be filled
out.
#2 We fill in the "To" and "From"
fields for you and mail it to the recipient.
If you go with #1, the shipping and billing address
on the order MUST be the same.
(We will charge your "shipping"
address, leave the cirtificate blank, and mail it to
your "shipping" address) See

address, fill out the cirtificate on your behalf, and
